VIENNA - Dr. Catherine Mandel from Melbourne, Australia, is the lead radiologist for the Radiology Events Register (RaER), a database of errors and adverse incidents in radiology that started in 2006 and currently has 4,000 cases. She discusses common types and causes of errors in radiology, and offers advice for avoiding them with AuntMinnieEurope.com Editor-in-Chief Philip Ward.
RaER is funded by the Australian government, the project is managed by the Royal Australian and New Zealand College of Radiologists, and the database is managed and administered by the Australian Patient Safety Foundation. But the database, available at www.RaER.org.au, is international and open to everyone -- radiologists, radiographers, referring physicians, nurses, and patients -- to report an incident.
