Malpositioned Intra-aortic Balloon Pump:
The IABP tip can be seen overlying the left hemidiaphragm (yellow arrow). The inflated balloon can be seen in the upper abdomen (black arrows). The tip is optimally positioned just distal to the take off of the left subclavian artery (ie: tip seen just at the inferior margin of the aortic knob). The balloon should not occlude the carotid and subclavian arteries at their origin from the aortic arch, or the renal or superior mesenteric arteries more distally (as would be the concern in this case).
